Any student wishing to participate in after school SOAR/Asset Grant physical activities must complete and return the Participation Agreement (link below).  Students wishing to participate must also complete the SOAR/Asset Grant Activity Sign Up Form (link below).  Hard copies of the forms are also available in the front office at San Pasqual and will be available in person at the SOAR activities.  However, forms do require a parent/guardian signature. So, anyone completing the forms at school will need a parent/guardian with them.
